Questions from administrators and directors of nursing

What does this cost the facility?

There is no facility fee. Consults are billed to the resident's insurance like any specialty consult, and there is no payment in either direction tied to access or referrals.

Who actually rounds in the building?

A physician-led team of MDs, nurse practitioners and physician assistants, working under a medical director double board-certified in physical medicine and rehabilitation and in interventional pain medicine. Your building gets a consistent clinician, not a rotation of whoever is free.

How often does the team round?

Cadence is set with each facility. Most buildings are covered on a standing weekly round, with new consults seen between visits as clinical urgency requires.

Which residents are appropriate to refer?

Residents with uncontrolled acute or chronic pain, refusal or poor tolerance of therapy, new or declining function, spasticity and contracture risk, neurologic deficits after stroke or injury, and complex or long-standing controlled substance regimens that need a specialist's review.

How does this affect MDS and PDPM?

Indirectly, and only through accuracy. Specific consult documentation — staged wounds, sided deficits, typed conditions, documented indications — means your MDS coordinator is coding from a clear record rather than reconstructing one. We do not advise on billing or case-mix strategy.

Is this telehealth?

No. Consults are performed at the bedside in your building. Telehealth has a place, but not as the substitute for examining a resident whose problem is physical.

How long does onboarding take?

Usually weeks, governed by your credentialing process rather than by us. An intro call, a visit, credentialing and chart access, then a first standing round scheduled with your DON.

What areas do you cover?

Skilled nursing facilities across the Los Angeles metro, San Fernando Valley, Ventura County, Orange County, San Bernardino County and San Diego County. Coverage decisions come down to drive time and round density, so a building just outside those areas is still worth a conversation.

Can you cover several buildings in our group?

Yes. Multi-facility groups are onboarded building by building on a common documentation standard, so results are comparable across the portfolio.
