Privacy policy
What this website collects, what we do with it, and what you can ask us to do about it. Short version: a contact form, standard server logs, and analytics only if you say yes.

Who this policy covers
Precision Pain and Rehabilitation Management, Inc. operates this website. The site exists so that skilled nursing facility administrators, directors of nursing, and regional leaders can learn about our consult service and get in touch. It is a business website, not a patient portal.
This policy describes what the website collects. It does not cover the clinical records of residents we treat. Those are protected health information governed by HIPAA and by our agreements with each facility, they are held in facility and clinical systems, and they never pass through this website.
What the contact form collects
If you submit the form, we receive the fields you fill in:
- Your name
- Your facility or organization
- Your role, if you enter one
- An email address or phone number
- Whatever you write in the message field
That submission is delivered to our business email and stored there. We use it to respond to you and to keep track of facilities we are in conversation with. We do not sell it, rent it, or share it with advertisers, and we do not add you to a marketing list without asking.
Please do not put resident names, medical details, or any other protected health information in the form. It is ordinary business email, not a secure clinical channel. If you need to discuss a specific resident, call us at (424) 301-7090.
Server logs
Our hosting provider keeps standard web server logs, which record the IP address making a request, the browser and operating system reported, the page requested, and the time. These are generated automatically by the server, are used for security and troubleshooting, and are retained on the provider's normal schedule.
Cookies and analytics
The site sets no advertising cookies and runs no advertising trackers.
When you first visit, a banner asks whether you consent to analytics. Your answer is stored locally in your own browser so we do not ask again. That preference is the only thing stored unless you accept.
If you accept, we load Google Analytics, which sets its own cookies and reports aggregate figures such as which pages are read and roughly which region visitors come from. We use it to understand which parts of the site are useful. If you decline, or simply dismiss the banner, no analytics script loads and no analytics cookies are set. You can change your mind at any time using the link at the bottom of the page.
Your rights in California
Under the California Consumer Privacy Act as amended by the CPRA, California residents may request to know what personal information we hold about them, request a copy of it, request that we correct it, and request that we delete it. You may also opt out of the sale or sharing of personal information — though in our case there is nothing to opt out of, because we do not sell or share personal information, and we do not use it for cross-context behavioral advertising.
We will not deny service, charge a different price, or provide a lesser level of service because you exercised any of these rights. To make a request, call (424) 301-7090 or use the contact form. We may need to confirm your identity before acting, and we will respond within the time the statute allows.
How long we keep things
Form submissions are kept as long as the business conversation is active and for a reasonable period afterward for our own records. Analytics data is retained on Google's default schedule for our property. You can ask us to delete your submission at any time.
Security
The site is served over HTTPS, so what you type into the form is encrypted in transit. Form submissions are sent over an authenticated connection to our mail provider. No website can be guaranteed against every form of compromise, which is the reason for the request above: keep protected health information off this site entirely.
Children
This site is directed to professionals working in skilled nursing. It is not intended for anyone under 16, and we do not knowingly collect information from anyone under 16.
Changes
If this policy changes we will post the revised version here and update the effective date above.
