Using this site
By using this website you accept these terms. If you do not accept them, please do not use the site.
This is not medical advice
Everything on this site describes a service we provide to facilities. None of it is medical advice, and reading it does not create a physician-patient relationship between you, or any resident, and Precision Pain and Rehabilitation Management, Inc. or any of our clinicians. No clinical relationship exists until a resident has been formally referred and seen.
Do not use this site for anything urgent. In an emergency call 911 or follow your facility's emergency protocol.
Not a patient portal
The contact form is a business inquiry channel. It is not a clinical system and is not an appropriate place for resident names, medical details, or any other protected health information. Please do not submit such information through it. For anything resident-specific, call (424) 301-7090.
Services are provided under separate agreement
Nothing on this site is an offer or a contract. Coverage at a facility begins only under a separate written agreement, and the terms of that agreement govern the service. Where anything here differs from a signed agreement, the agreement controls.
Figures and statements on this site
Where we state results, those are historical figures from facilities we have covered, measured against each building's own baseline. They describe what has happened in specific buildings under specific conditions. They are not a projection, a promise, or a guarantee of what will happen in any other building. Clinical and operational outcomes depend on the resident population, the facility's own staffing and processes, and factors outside our control.
We try to keep the site accurate and current, but we do not warrant that every statement is complete or free of error, and we may change the content at any time without notice.
Our content
The text, layout, logo and other material on this site belong to Precision Pain and Rehabilitation Management, Inc. or are used with permission. You may read it, print it, and share it internally within your organization for the purpose of evaluating our service. You may not republish it, sell it, or present it as your own.
Acceptable use
Please do not attempt to breach or test the security of the site, submit automated or fraudulent form entries, scrape it at a volume that degrades service, or use it to send anything unlawful.
Links to other sites
Where we link out — to CMS material, for example — we do not control those sites and are not responsible for their content or their privacy practices.
Disclaimer and limitation of liability
The site is provided as is, without warranties of any kind, express or implied, to the fullest extent the law allows. To the fullest extent the law allows, Precision Pain and Rehabilitation Management, Inc. and its clinicians and staff are not liable for indirect, incidental, special, consequential or punitive damages arising from your use of this site, or for any decision made in reliance on it.
Nothing in these terms limits any liability that cannot lawfully be limited, and nothing here alters the obligations in a signed services agreement.
Governing law
These terms are governed by the laws of the State of California, without regard to its conflict of laws rules. Any dispute arising from this site will be brought in the state or federal courts located in Los Angeles County, California.
Changes
We may revise these terms. The current version is always the one posted here, with the effective date shown above.
